Life Insurance
You are provided a $10,000 premium-free life insurance benefit. The benefit is payable to your Beneficiary(ies) upon your death, for any cause, while you are covered under the Plan.An Accelerated benefit of up to $5,000 is payable prior to your death if you have a terminal illness and you are expected to die within six months. The Life Insurance Benefit will be reduced by the amount of the Accelerated Benefit.
Beneficiary Designee Card
If you are not sure if you have your Beneficary Designee Card on file or who you have named as your beneficiary(ies), please complete a new Beneficiary(ies) Designee Card.
The Beneficiary(ies)Designee Card can be downloaded by clicking on Documents at the left side of our webpage.
Without a beneficiary card on file, your life insurance benefit would be paid according to the Classes of Successive Preference Beneficiaries.
