Some COBRA participants are eligible to extend their dental and vision benefits.

If you and/or your dependents qualify for this extension, the Fund will notify you in writing.

This extension of dental and vision benefit coverage must begin immediately following the expiration of your COBRA coverage.

Extended Self-Pay participants are responsible for paying the following monthly premiums by the first of each month to ensure eligibility for benefits.
